ADHD Assessment Battery

Adult ADHD screening battery integrating ASRS v1.1, PHQ-9, GAD-7, MDQ, ISI, WFIRS-S, and DSM-5-TR/ICD-11 diagnostic context checks. Results organize ADHD symptom support, impairment, developmental/cross-setting evidence, and differential-review flags; they are not a standalone diagnosis.

This battery is a structured screening and clinical-organization tool, not a diagnosis. It brings together adult ADHD symptoms, functional impairment, developmental history, cross-setting pervasiveness, and common differential considerations.

Complete each section using its timeframe: ASRS v1.1 uses the past 6 months; PHQ-9 and GAD-7 use the past 2 weeks; MDQ asks about lifetime periods when mood or energy were not your usual self; ISI uses the current/last 2 weeks; WFIRS-S uses the past month. Use N/A on WFIRS only when an item does not apply.

The final report integrates ASRS Part A threshold scoring, PHQ-9 depression and self-harm risk, GAD-7 anxiety severity, MDQ bipolar-spectrum screening, ISI insomnia severity, WFIRS-S impairment domains, and DSM-5-TR/ICD-11 context indicators. Clinician review is required before any DSM-5-TR diagnosis, ICD-11 code assignment, medication planning, or treatment conclusion. If PHQ-9 Item 9 is endorsed, safety assessment comes first.
